Cynthia C. Copeland, DDS, DIBDM, AIAOMT Performs Chao Pinhole® Surgical Technique in Santa Barbara, California

Pinhole procedure restores gums to their best function and appearance

The Chao Pinhole® Surgical Technique (PST®), invented and patented by John Chao, D.D.S., is a scalpel-free, suture-free procedure for treating gum recession.

This procedure is performed by making a small hole with a needle in the gum tissue. Using specially designed instruments, the gum tissue is loosened and guided over the receded part of the tooth. Since there is no incision or suturing, patients can expect minimal post-operative symptoms (pain, swelling and bleeding). Most patients also are pleasantly surprised by the instant cosmetic improvement.

*interlude music* many people think a trip to the dentist is all about your teeth but healthy gums are just as important. For Roxio her gums were not only a health issue but an embarrassing problem as well. When people think of me probably the first thing that comes to mind is my smile and my laughter. Underneath my smile there are a lot of insecurities. I learnt to smile by showing only half of my teeth – I noticed that half of my gums were receding about 5 years ago. I have straight teeth so my gums are really bad. I have asked dentists if I have gum disease and he said no it is from over brushing and brushing to hard and I caused a lot of damage to my teeth and it does bother me a lot. My daughter is getting married in May and we are going to be taking a lot of pictures and I want to smile really nice for my daughter’s wedding. I want to express my happiness through my smile. Now since Roxio worked in a dentist’s office she knew she had to fix her gummy problems but her gum skin grafting surgery was just too daunting. That is why she opted for a brand new procedure. Hello Roxio! Hello Dr. Chow how are you today – how are you today? I am doing well. Good good – by tomorrow the gums will be right about ere. A pin hole gum rejuvenation is the first minimal invasive technique to restore receding gums without the gum graft. With this procedure we can move gums to it normal position without cutting or stitching all though the tiny pin hole. We are about to start to the procedure with a pin hole. Right at this point this is the instrument that I use, it goes through the pin hole I am loosening the gum from the attachments to the bone underneath and to the root underneath and it just kinds of glide down and I am going to move to another tooth. Now I am using a longer instrument so I can reach Roxio’s back teeth it is going pretty quick. The pin hole will disappear in 24 hour, and this is all in one sessions and I am going to be done soon. Now I am going to put another pin hole right over the upper left tooth and we have about 3 more teeth to go. Now I am moving towards the last 2 teeth, and so far I have used only 2 pin holes. Now we are ready to put the collagen strips in between the teeth and then we will be done. The collagen strips will keep the gum from going aback up. Now the gums are right where they belong and we did it in 57 minutes. We did 10 teeth – and it looks really nice here. A total change. Ready? Yeah. Oh my God – this looks so different, I am sorry! I am so happy! I am so excited. (applause) Roxio is here with us today and that was an emotional moment for you was it not? Yes it is amazing I can’t believe it. You show that smile off. Just smile to that camera because this is a transformation that goes beyond just a healthy mouth this is emotionally a big deal for you. Wow. You know what amazing is that we are taught to brush our teeth 2 a day but we are never taught about how hard or soft to brush our teeth and gum recession is a serious thing and people get confused because you want to prevent cavities, and yet you can’t over brush them because them you are going to cause gum recession. Roxio’s doctor Dr. John Chow is joining us now on the phone – Dr Chow this is something that you came up with and it seems too good to be true in a way because you are essentially pulling down the gums and within an hour or so, you have treated the receding gum line – walk us through exactly what you are doing during the procedure. I went  from the top of the tooth to the area where the tissue is very soft but very very viable, and full of collagen so when you introduce the pin hole and then I basically just engage the tissue from the underlying bone and then I was able to physically bring the gums down to where I wanted it go to, and then I stabilize it with the collagen strips and put it between the teeth and that is about it – the secret is to go from the soft tissue it is easier to close the tooth. Dr. Chow thank you so very much for sharing this with us. And everyone at home reverb it is a fine line between healthy clean teeth and over brushing with hard bristles so be careful use a soft tooth brush if you see receding gum lines that is another reason to go see your dentist. 2 a year to make sure you stop problems before they become big problems, let’s see what coming up next.

Live from ABC 7 his is eye witness news. As we get older our gums tend to wear away and expose most of our teeth. This can make you more vulnerable to disease causing bacteria and gum surgery can be painful. Specialist Denise Denor is treating receding gums that is practically painless. Chances re if you have sensitive teeth you could be suffering from a reseeding gum line, this is usually the result of gum disease or aggressive brushing. 50 percent or people have some form of gum recession. A painful surgery with a long recovery used to be the only answer. Now a local dentist is trying to ease the patients and millions of others – no surgery no sutures all smiles. That was what Dr. Chow wanted for his patients. I just don’t see why dentistry has to have a procedure that hurts so much. Conventional gum grafting is painful. Dentists cut and remove tissue from the roof of the mouth and then graft it onto the receding gum. Now Dr. Chow has developed a new and almost painless procedure called pinhole surgical technique. There is recession of the gums going up this way. Maria Viscera is getting the procedure today. There is some sensitivity! Dr. Chow doesn’t cut but uses a small needle to make a small hole in the diseased area – a specially designed small area to freeze the tissues from the bone, when the gums are moved and bound to a healthy position, Dr. Chow then places collagen strips under the gums to keep the tissue in place. We will loosen up the gum and let it down to where it is supposed oth be and enforce it with some collagen. The pinhole surgery can correct 10 to 14 teeth at once. It takes about 20 mins for up to 3 teeth and recovery is less than a day. Traditional surgery requires 2 incisions and only correct 1 2 teeth at a time, takes almost 2 hours and there is a 3 week recovery. Joy was one of the first to have it done 3 years ago. This was Sandy before the pin hole procedure and the day after. It was a miracle – and absolutely miracle. Dr Chow says he has not seen any higher risk with the pin hole technique than with the tradition method the cost is comparable to traditional methods that can range from 500 dollars to 1800 dollars. Right now Dr. Chow is the only dentist who is doing this procedure but he is beginning to train dentists around the world. Denise Denor eye witness 7 ABC

NBC for news at 5 starts now. Speaking of issues do you wish your smile was better because your gums are receding? I will tell you about a new non-surgical cure. Okay now we are going to our own Dr. Bruce Hansel and if you have a problem with your smile he has as solution for you – Dr. Bruce. That’s right the solution Chuck is a lot better than what other people have heard about in the past and most people think about their teeth when they think about their smile but the bottom line is in many cases it is beau of the gums, too much too little, receding or discolored or as I pointed out – painful surgery used to be the only way to fix that until now. Loraine Jackson over her smile now but this is what it looked like one hour before. I had some receding gum lines and some bone loss, and I was not used to not smiling all the way. This affected Loraine’s self-esteem and income. Out of work she has to go for many job interviews without a confident smile. You always want to look your best and feel your best – a recent study showed these gums could endanger her health. The bottom line was that Loraine needed a new smile but she did not want extensive surgery and take a long time to recover from. Hi! How are you – so she visited dentist John Chow who offered a unique procedure called pinhole gum rejuvenation. It doesn’t require any cutting it doesn’t require any suturing or stitching and it doesn’t require any grafting of tissue from one part of the mouth from another. When Lorraine heard that she jumped at the chance. I got very emotional because I said well this is the answer to my prayers! Her is how it is i done. Th doctor makes a tiny pin hole in the gums above the teeth and using special instruments go through the hole and release the gums. Basically the receding gums back to their normal positions. It takes a very low time you don’t have to cut it you don’t have to suture it – while it took only 20 minutes the whole mouth and hour to an hour and a half. It is quick it is easy and it is instant the gums look normal and healthy – all of a sudden the teeth also look healthy and normal, and young again. Go ahead and take a look let me help you to pull this up a bit. Oh wow! So 64 minutes later it looks like this from what it was before. Here is Loraine before the procedure. Here she is now. Some people dream of wanting to win the lottery, but if I can have something medically to preserve my health and my teeth – that was just a joy for me. Now if you want to improve your smile your teeth and gums can both be looked at by an expert to determine what exactly you need, now as I said that may include teeth as well as gum work – whatever you choose though. Make sure you see someone experienced for the procedure and speak of former patients about results to get benefits without risks.

Receding gums don’t just affect older people. The American dental association says that half the Americans between the ages of 18 and 65 already have some degree of gum recessions and the treatments are usually a gum graft, but now there is a new less invasive procedure. Dr. Chao and Dr. Goldman join us now live with a look at that – good morning to you too. Good forming! First let’s start out with why people should care about their gums and receding gum lines. Well one of the things that really affect people when their gums are not in the right place it can affect the smile, peoples live are actually affected because you just can’t smile or feel confident and of course when there is gum recession, there is a sensitivity sometimes, and if it is not treated it can cause eventual tooth loss. Oh. So why these people are caught in a dilemma they have a problem but to fix it is kind of drastic. There is the gum grafting technique which calls for us to cut the gum from the pallet form the roof of the mouth and stitching it to the place where it is needed, and so being caught between having a lot of pain and just keeping with the smile is not so attractive, some people go and bare it, get it done or just live with a bad smile. Well a lot of people don’t like going to the dentists – here he is. Being having gum surgery and cutting it open right out – Jesus just thinking about it just gets me – you know. So now we have a quick easy and huggable way. A huggable way (laughs). Using this is like surgery without surgery. We can move the gums down through a pinhole and we don’t have to cut, we don’t have to sew. Wow! And the results are instant. If you have gum receded and when we are done with it in an hour or so it’s back down where it is normal and there is no stitches there is no evidence or cutting and it is an instant fix. One of the most exciting thing that has happened to dentistry in the last 25 years. Yeah that is huge. Can you compare the old way vs. the new way. Yeah the old way you cut it form the pallet and you stitch it, with this way we go through a pinhole with special instruments, I am generally not as nervous as that. (laughs) yeah! Ok so now this is the old way, it is kind of replaying. Oh y goodness! Yeah so that is the old way and this is the new way and it works really really well. Oh my gosh! And it is something, that we make sure to stay that way with collagen. Oh so you insert collagen after that. Yeah so we don’t have to do stitching. Dr. Goldberg, you know you have brought this treatment method into your office. Why did you choose to do so. Well just like Dr. Chao mentioned you know recessions – it’s a huge issue that I see in my practice, and I practice with my father Joe Goldenberg and for years, we have been referring patients to have their traditional procedure done for the grafting and most of the time they would never go. They are afraid that it is going to be painful or if they did have it done, they didn’t want to have it done again. Mhmm. So once we found about the pinhole gum rejuvenation technique, that Dr. Chao invented we were thrilled to offer this to our patients. actually most recently we had a patient named Linda and she was in her 50s could not smile anymore, she was told she needed the grafting done, and she didn’t want to so she went online and fond that we offer this procedure and she is thrilled, she cannot stop smiling actually immediately after the procedure we gave her a mirror and she looked in the mirror and she almost stated crying. Wow she could not believe it that for years she could not smile and she was just happy because instantly it was- looking exactly how she wanted to like nothing had ever happened there was like I said no cutting gum stitches, like she never even had surgery. Wow! It was pretty amazing. So she is one of the many people that go through this, Dr. Chao can you please explain what a patent goes through when they have this technique for those that might want to go get this done. Yeah well it’s quite simple because the gums are numbed up and we would pt. a tiny little pin hole, above where we want to treat it, and you specialize instruments that I invented and basically you lift the gums from the underlying structure and can bring them down to where it belongs, and then because it is still a little loose, so through the pin hole then I will put collagen underneath it and then kind of stabilize the gums, where it could go and because there is no stitching on it, there is no evidence of cutting when the patient smiles, it looks like the gums are in the right place. Mhmm, and so it looks like tis i total rejuvenated. It is like surgery without surgery. Wow! That is what pianists say – so we do get a lot of hug. It is quick, and that is easier on the patient because there is hardly any pain, and hardly any down time – yeah totally. It is huggable because we dentists would do it – about 150 of us by now in the world, and we get a lot of hugs. And we have a before and after picture that I believe that we can show- look at that! Wow! That is on the left hand side we did the full mouth upper and lower in less than one hour. Oh my goodness! Upper we did this and in less than one hour – it changes people’s lives when this kind of thing – that is awesome. When it is done for the patients. They often say it is surgery without surgery, Mhmm. This is like a miracle they will say! It’s like magic Dr. Chao! How do you do this? We are so thrilled that we can do this method to the public, and to the dentist who are trained for this. I am sure there is different prices everywhere, but Dr. Goldberg for instance, quick question – quick is a question here. How much does it cost and is it covered by issuance at your office? It does, and varies depending on insurance that you have, but most insurances do cover a portion of it and it is usually done by site so it can vary from about a 1000 and kind of works its way up. Ok, but definitely right around the same range and the traditional procedure. Exactly, actually because we can do so many at one time we can actually charge much less, than the grafting. There is much less down time for the patents too. Dr. Chao Dr. Goldberg, thank you so much for the mooring. This morning I feel so excited about that. New opportunity to do surgery without actually doing surgery. Yeah exactly! Thank you!

Receding gums don’t just affect older people. The American dental association says half of American between the ages of 18 and 65 already has some degree of gum recessions. Take a look here – causes include improper tooth brushing, gum disease and even wearing braces. Tooth sensitive and tooth loss can happen if this is you, you may have avoided having gum grafting surgery because you believe it is too invasive and painful. Well after years or research dentist, and inventor Dr. John Chao had developed and easier way to correct receding gums. Chao pinhole gum rejuvenation is a minimally invasive technique that corrects receding gums through a tiny pin hole as you see here the perfect final result. Dr. John Chao joins us now as well as Doctor Ed De Andrade a periodontist in Vegas and Henderson who is a provider of the Chao pinhole gum rejuvenation in our area. Good morning to both of you! Good morning and thank you for having us. Now Dr. Chao let’s start with you – why should people care about receding gums? Now as you mentioned there are 2 reasons one is hypersensitivity. Where the gum recede from the teeth, roots are shows and these roots can get very sensitive and they can impact the quality of life, and some people can’t even drink anything cold, that is one reason – the second reason is that, loss of gum needs less support for the teeth most of all, and the roots get exposed that can get decayed – so it does not interrupt the position of losing teeth in the long run. But the strongest reason is that it is not very good looking. It could be a – it can make a person look when thy smile with the gum line irregular and the roots exposed and it could have a real strong quality of life fort because  sometimes people don’t want to smile they lose confidence. They are embarrassed right? Yes they are embarrassed they don’t want to smile thy want to cover their mouth up when they smile and I actually happens, they don’t t you see which is a little more serious than they need to be and so, and that when it gets done when we correct – in self-grafting from the roof of the mouth what we can correct with is a pinhole gum rejuvenation technique we can do it quickly, easily and instantly correct a problem, because we don’t use any cutting we don’t have to stitch it, we do everything through a pinhole. And in conventional gum grafting there is a lot of cutting? Yes can we take a look at some of the videos than that we are showing right now? Describe to us what we are looking at. This is gums being cut in the pallet of the roof of the mouth, and the flap is made on the outside and the tissue from the roof of the mouth is stitched to the gum, where it is needed, when the pinhole gum rejuvenation technique then we put the pin hole into the gum, above the recession and with specialized instruments that I invented then remove the gum gently down, loosen it up and then make it so loose that actually it drapes over the teeth in very gentle way again without cutting or stitching, and this is like many there procedures in medicine, where things are done through, subterranean way where actually you don’t see, there is not cutting. This is quite a medical breakthrough for people who want this medical procedure done who are afraid in the past to mile and show their teeth and get them back to a normal healthy gum level. Yes so this is something that I believe is a breakthrough, alleviating people’s fear of getting this corrected because they don’t want grafting and the news is out about the grafting procedure casing a lot of havoc with patients and a lot of downtime problems. Now Dr. De Andrade why did you decide to offer this procedure in your clinic and office? Well fear definitely was the main reason, we have been diagnosing recession and lack of gum on patients for many years, and realize that patients understand that they have a problem and they realize that they need to have it done for many reasons that Dr. Chaos explained earlier, but tooth loss is a problem associated with recession that progresses. So fear was my most highest driving force behind me adopting this procedure, because it is very simple and painless patients are surprised after I am done, it took so little time, and that is really the main reason. People talk to their neighbors they hear about me taking tissue form the roof of their mouth. And they are completely paralyzed. This seem minimally invade. Oh absolutely. Essentially called a huggable procedures. A huggable? Huggable – oh huggable! We get hugs now. Oh! Really? Wow that is! We get smiles and thank you’s like there is no surgery and paints look at it and it is down where it belongs and it is where it was without stitching, without nay incisions on it, they actually hug us – we get a lot of hugs – that is sweet! Is this just one visit typically? Oh yeah that’s it? By the end of the procedure you can see it? Less than an hour. Oh? Yeah you see that the other screen – ok, you see how it is treated on the left side. This is a full mouth and we did that one in about an hour and a half, and immediately when you come out it looks like that. Typical result her? Exalt right there. This is a 9 year case. 9 years later it looks like that – wow! So there is hope for people. So this is a he, he got married 6 months after that (laughs) he was smiling really big! So someone who has had this problem for 10 20 years you can still go and have this procedure done? Yes it can be very drastic – but one appointment and it is down to where it all belongs. Absolutely. We do 1 2 teeth just like the before and after were showing several teeth on the upper and lower jaws, but it has definitely changed my practice. We are getting people from all over the state that have looked us up online, and they want to have this procedure done as I tell people, it is – I am excited about it because these people were diagnosed and they know they have a problem and now they know there is a minimally invasive way of resolving their issues. But Dr. De Andrade didn’t you say that you advised a patient to bring a box of Kleenex? Yes well because they tend to get so emotional afterwards, and that is what I said. We were joking! It’s a joke I do tell them at the consultation to bring some Kleenex because you are going to try when you see the results. It is pretty life changing. Really quickly let talk about the training that you offer to the dentist. Yes I have trained over 850 dentists, most of them are in the US and Canada. But they come from all over. Last weekend we trained people from Mongolia. Wow all over the world. They come from Australia different parts of the world. And he is the only dentist here in town – a periodontist that does this procedure? I am the only periodontist – board certified periodontist. It is going across the country. Well thank you so much it is such an eye opener with the before and after it is incredible. It is great, thank you so much thank you for having us. KEYE TV’s Austin

KEYE TV’s Austin

Alright everybody welcome back to we are Austin, we are letting you know how you can get those pearly whites shining bright yet again. Having a receding gum line now can ace a person look older and lead to tooth loss and make it difficult, so myna people avoid getting their gums fixed because it might be a little too painful. That is right, one dentist developed a way to fix receding gums using a minimally invasive technique. Joining us are Dr. John Chao and Dr. Corrine Scalzitti thank you both! Goo morning so nice to be here, it is nice to see you! It is so nice to be here to talk about something like surgery but is not surgery. Right! Exactly so Dr. Chao you invented this procedure. Before we get into the specifics, let’s talk about receding gum line, why is it even important? Well receding gum line means you are losing bone and gums, and it can lead to decay of the roots and eventually loss of teeth, but it is all preventable – up to now there is the grafting type – you take a piece of gum from the roof of the mouth and then tag it on the way needed. Right – but now we can do it differently and this is huge – this is the other tenure the conventional one where people don’t want to go for. Right, they don’t want to go because they cut the gum and put that piece of gum right over where they need it. People are afraid of that. Yeah so now I have invented a technique where we go through a little pin hole in the gum, so this is completely different. Completely different because it doesn’t involve cutting we don’t use stitching, through a little pin hole with a special instrument which is taught to 800 dentists across the country, and we can move the gum line down. we can move it form the binding underneath and slowly let it come down to where it belongs, and then when it is down we can through the pin hole insert collagen strips there and then when that is set and done you can hardly tell tis i done. It is like gums has grown in just one appointment. And it doesn’t go back? Oh wow! It doesn’t go back and it is a permanent fix. And you are saying how this is minimally invasive. What did you call it? I call it a huggable procedure! A huggable rocker! It is a procedure that gives us a lot of hugs (laughs) because people are very happy! Yeah you don’t do surety generally and get hugs. But we do with this one because it looks so natural and it is really an inspirational thing to do for the patient. And I know Dr. Scalzitti you offer this to your paints here at Austin? Why did you decide to start doing that – and what has the response been like? I was so thrilled when I heard of  technique different from grafting that I couldn’t wait to take the course and Dr. Chao is the most wonderful teacher he just makes sure you do it his way when you walk out the door so you are very competent. Yeah – you have some paints you want to tell us about some of them? I have wonderful patients one we have Jason who has an infectious personality and he was working abroad and he was going back the next week and he heard about it he wanted to do it immediately. So we did and I got to see him one day afterwards and he had this huge grin and he said this is my money smile. wow and that is so great and of course that made you feel so great about the whole thing. Did he give you a hug afterwards? Like most? I got a hug and then on the way back to Iraq he managed to order a fruit bouquet for us! That is so sweet! There is the huggable procedure. Very huggable. There is another patient we took care of recently – Lisa acme in she had needed it for year and she knew that she did and had avoided it because she was worried it would be painful and she would have a lot of downtime. She came in and when we handed her mirror at the end because the results are instant. When we handed her mirror she had tears in her eyes and then she just could not stop smiling. With tears in her eyes, and Dr. Chao you said you can actually eat later that day. Yes, you can eat – you are good to go. There is no pain right afterwards and it looks normal the next day so we have the before and after pictures lets go through some of these, you can actually see the receding gum line there – yes that is all done in one day, and you can hardly tell because we don’t cut it we don suture it we don’t stitch it =, and this is a full mouth case upper and lower. We were able to – why do we even get receding gum line? Why does this even happen? Over brushing gum disease, and a lot of times it is over brushing. It sounds like you have been training doctors all over the world in this procure. If someone want sot find how to locate a doctor her in Austin who is doing this, how do they do that? They can go to my website pinholeacademy.com and find dentists there. They are certainly welcome to Dr. Scalzitti – of course she is making patients very happy day in and day out. Yes! We would be delighted that would be great! Well thank you guys so much and of course there is all the information you need on the screen, thank you for sharing your story and thank you for making people smile and get their confidence back. Dr. Chaos thank Go – now I must say you have a problem too! I do we were talking about my doctors that – Jordon there is an area in your mouth that we are keeping an eye on and every Tim I go we have to check it and I get worries. Can we do it now? (Laughs) I don’t want to do the gum graft because it scares me but now I know what to do! We will be right back after commercial break how does that sound? Thank you Dr Chao and thank you for being here.

Frequently Asked Questions

Gum recession refers to the loss of gum tissue along the gumline. This can occur as a result of periodontal disease (gingivitis, periodontitis, advanced periodontitis), the natural aging process, or abrasive habits when it comes to brushing the teeth.
When gum recession occurs, the root structure of the tooth becomes exposed. This means that tooth decay and other problems can affect the teeth along the gumline and beneath it. Since healthy gums are essential for a healthy mouth, getting gum recession treated is important for lasting dental wellness.
The Chao Pinhole® Surgical Technique is a minimally invasive option for treating gum recession. Unlike traditional grafting techniques, PST is scalpel and suture free. All of the tools and techniques used to perform the Chao Pinhole® Surgical Technique were created by Dr. John Chao.
Traditional gum recession treatments involve the use of donor tissue or soft tissue grafts in order to rebuild the gumline. This soft tissue would be sutured in place and would join with existing gum tissue as it healed. While this traditional grafting treatment is effective, comparable results with better patient experience can be achieved through the Chao Pinhole® Surgical Technique.
During the Chao Pinhole® Surgical Technique, a needle is used to make a small hole in the patient’s existing gum tissue. Through this pinhole, special instruments are used to gently loosen the gum tissue. These tools help expand and slide the gumline to cover the exposed root structure. There are no grafts, no sutures, and no incisions needed with the Chao Pinhole® Surgical Technique. It simply involves the adjustment of the existing tissue.

The benefits of the Chao Pinhole® Surgical Technique are many:

  • Less Discomfort for the patient after treatment.
  • Faster recovery for the patient than traditional grafting.
  • No need for uncomfortable sutures.
  • No need for scalpels or invasive surgical tools.
  • No need to take donor tissue from the patients palate.
  • Excellent, natural-looking, long-lasting results.
